Abstract
The Medical Records System is one of several subsystems that make
up a Medical Information System. It plays an important role in the
development of Medical Sciences, and in planning individual patient care.
So far these records are usually maintained manually. Naturally, it has
severe limitations. In this dissertation a11 such limitations has been
identified and a computerized medical Records System have been proposed.
Obviously, it is relatively more reliable, economical and faster.
In addition, it is capable of offering much more than the current system.
The proposed system has been tested on live data collected from Ahmadu
Bello University Teaching Hospital, Zaria. The programs and their outputs
have been included towards the end of this dissertation with the processing
details fully explained in the text. The system comprises of essentially
three subsystems. They are: Data Check and Store (DCS) subsystem, which
is designed to store the coded information on treated in-patients in two
different files after record validation one of the files is an active on
stored on magnetic disc for frequent processing. The other having a
greater records length is infrequently used and is stored on a magnetic
tape kept away to be activated and deactivated according to needs.
A second subsystem, Report Genera ting (RG), is responsible for producing
both the reports that are manually possible, and certain others
that are not being produced by the manual system due to size and
complexity even though they are qually desirable.
To cope with the growing number of recuests for patients' data
necessary for research, an interactive Health Inquiry (H..I) subsystem is
introduced to handle inquiries on the frequency of any of the 999 major
disease groups for a given period and their associated mortality counts.

As far as the information goes this attempt is perhaps the first of its
kind in the country.
The Medical Records Department of ABUTH, Zaria is now all set to
implement the proposed system and the Medical Reoords Department of
ABUTH, Kaduna will soon follow suit.
